At least one of the members on my forum is being asked for a confirmation code every time they want to post, I have tried looking for the problem both on my forum and this one without any help.
I can't think of anymore information to add and hope for a solution as soon as possible, thank you.

Hello,
The user might be typing words that are hitting the censor list and will make the user enter a confirmation code. There isn't away to prevent this other then make sure you don't type spam, sexual or harmful words.
